71 year old female underwent WB PET /MRI as part of the initial staging workup for Ca right breast.

Sagittal STIR image (A) showing a tiny hyperintense lesion in the L2 vertebral body(arrow). Corresponding Sagittal PET (B) and PET/MRI fused (C) images showing mild focal uptake (white arrows). There was a gradual increase in the size of the lesion over two years as seen in the serial MRI in 2015(D) and 2016 (E) (red arrows). Subsequent CT guided vertebral biopsy suggested Metastatic Adeno Carcinoma.
